Year-to-Date Information
- Current Quarter Reports
- Submit a Payroll Summary
for each payroll you've run in the current quarter.
Date Range = Check Date of payrolls run in current quarter.
- Year-to-Date Reports
- Submit a Payroll Summary
for each quarter this year.
Date Range = Quarter Date Range of payrolls run in previous quarters.
i.e.: For quarter 1, enter: Jan. 1 through March 31.
Submit Your Reports in One of These Three Ways:
Once you send your reports, a SurePayroll representative will be in contact with you to
set up your first payroll.
- Fax your reports to 847.676.5181. Be sure to include your phone number or e-mail
address. You can print the attached cover sheet.
- E-mail your reports to info@surepayroll.com.
- FedEx your information to us for free. Call 1-888-403-7738 and we'll provide a shipping
label and arrange to have your reports picked up at your location — on our dime.
